A Purple Heart may only be given for injuries sustained in combat. In the Army we have the Soldiers Medal which is used to reward a Soldier for actions which if had they occurred in combat would have resulted in the awarding on a Bronze Star. So if a Soldier saves a person from a burning car at risk to his own life, The Navy doesn't appear to have a "Sailors Medal" so they would likely use an commendation medal or a Bronze Star for this.
